How does it work?
On contacting our service our admin team will arrange for a senior member of our team to visit to complete an initial assessment.

What happens at an Initial Assessment?

Our qualified staff will go through and take your personal details and ask you a set of questions relating to your reason for contacting our service, your current abilities and what you would like us to help and assist you with, they will discuss setting aims and goals that you feel will be realistic for your capability.

Risk Assessments will be completed, these are to ensure the health and safety of both our service users and staff. Once the assessment is complete our staff will return back to the office to complete individualised care plans.

Service User Records

A set of personalised records will be created on our system app, these records will contain relevant information to the care package that has been chosen by yourself, these will also include details of how we will be supporting you. It will also contain the risks assessments that have been completed. Your records will be updated regularly, staff will record each visit on the app. We welcome our service users and families to read through our records and if there is anything you would like to discuss further you can contact the office or speak to the carer that is visiting you.
